Patient Privacy and Consent to Collect and Store Information

Dr Richardson's practice is bound by the Health Records and Information Privacy Act introduced by the NSW Government in 2002.

The legislation sets the standard by which we handle personal information collected from patients. As part of our commitment to providing quality health care, personal information is sought from you in order to provide a proper assessment, diagnosis and treatment. Your personal information may therefore be disclosed to other health care professionals involved in your care and your medical file will be handled with the utmost respect to your privacy.

When you attend for your first consultation you will be asked to sign the following declaration:

I _____________________________, have read the above and agree to allow Dr Richardson to access relevant information about my medical condition. I give permission for my details to be collected and to be used in communication with other health professionals involved in my care as well as ancillary services. I also give permission for my medical condition to be discussed at any multi-disciplinary meetings in relation to my treatment at Dr Richardson’s discretion. I also understand that relevant staff of Dr Richardson’s practice may access my clinical records.
